As part of our ongoing commitment to closing the digital divide in healthcare access, pathway offers the following core services designed to support community members in navigating virtual care options confidently and independently
In-Person Support
Ensuring Digital Continuity of Care
At the Care Connected Lounge, we offer D.C. residents in-person assistance to set up remote monitoring devices and effectively use their digital health tools. Whether you need clarification on a new remote patient monitoring order right after your physician visit, or wish to proactively track your health data with digital devices, the Lounge is available.
Community members can walk in to receive hands-on support with installing and configuring remote health monitoring devices, accessing and operating telehealth platforms, and addressing other digital health-related questions.
Our trained staff is available to answer technical questions related to these devices and platforms. This service is educational; participants are encouraged to come prepared to engage and learn.
Telehealth appointment support
To ensure that lack of internet access or personal technology is never a barrier to care, we provide a private, tech-enabled space for D.C. residents to attend their virtual doctor’s appointments.
Residents can call us to schedule their telehealth visit and reserve a dedicated desk at our Connected Care Lounge. We ask that residents call ahead to book their time slot and arrive 30 minutes early to allow ample time for setup and troubleshooting, ensuring a smooth connection for your appointment.
If you are unable to attend your scheduled appointment, please notify us in advance. This allows us to offer the spot to another resident on our waitlist.
Digital Skills Training
We offer instructor-led workshops that help D.C. residents build digital skills for using health-related technology.
These sessions provide hands-on training to navigate telehealth platforms, communicate with providers, access test results, manage follow-up instructions online, and understand how to use wearable devices and remote patient monitoring tools.
By fostering digital confidence, our workshops support long-term engagement in virtual care and help improve health outcomes across the community.
